健康保険において、被扶養者が引き続き要件を満たしているかを確認するため、被扶養者資格調査を実施します。

・本調査の対象となった方は必ず対応が必要です
・7/27(月)にEY健保から対象者へメールが送信されます

・期限までに提出がない場合、ご家族はEY健保の加入資格を失い、扶養から外れます

１６歳以上の被扶養者（令和７年６月３０日以前に扶養追加となった者に限る）

※海外赴任中の被保険者の「子」と「配偶者」は対象外

To confirm that dependents continue to meet the eligibility requirements for health insurance coverage, a Dependent Eligibility Review will be conducted.

・If you are selected for this review, you are required to complete it.
・On Monday, July 27, selected individuals will receive an email from the EY Japan Health Insurance Society.
・If the required documents are not submitted by the deadline, your family member(s) will no longer be eligible for EY Japan Health Insurance coverage and will no longer be covered as dependents.

Please check the email you receive from the EY Japan Health Insurance Society on Monday, July 27, and submit the required documents.

Initial login deadline: Sunday, 2 August

Submission deadline: Monday, 31 August

■ Who Is Required to Complete This Review?

━━━━━━━━━━━━━━━━━━━━━━━━

Dependents aged 16 or older (limited to those who were added as dependents on or before 30 June 2025)

*Children and spouses of insured individuals on overseas assignments are not subject to this review.
