　日頃より、健康保険組合の業務運営にご理解とご協力を賜り、誠にありがとうございます。さて、表題の件につきまして、当健保組合より以下のとおりご案内申し上げます。

１．資格確認書紛失時の再交付有料化

　「資格確認書」の再交付手数料について、2025年7月24日の第41回EY Japan健康保険組合組合会で承認され、2025年12月2日より有料化することとなりました。罹災による紛失を除き、再交付時に手数料を徴収いたします。

（１）実施開始日

　2025年12月2日（火）（本人申請日が12月2日以降より適用）

（２）手数料

　1人につき1,000円（振込手数料はご負担ください）

（３）資格確認書再交付の手続き手順

　①「資格確認書再交付申請書」に必要事項を記入してください。

　②指定口座へ、被保険者の戸籍名で組合の口座に再交付手数料（1,000円）をお振込みください。

　※振込控えの添付は不要です。

　③記入済みの再交付申請書を事業所委託先宛てに送付してください。

　※任意継続の方は、直接EY Japan健康保険組合宛てに送付してください。

　④健保組合にて再交付申請書を受理後、内容に不備がなければ資格確認書を再交付します。

　⑤再交付された資格確認書は、事業所委託先を経由して、ご本人様の住所宛に簡易書留にて発送され

　　ます。到着には2～3週間ほどお時間を要します。

（４）注意事項

　・罹災の場合は、再交付手数料を免除します。再交付申請時に罹災証明書の添付が必要です。

　・再交付申請書を健保で受理し、振込を確認でき次第再交付いたします。

　・領収証の発行はいたしません。必要な方はEY健保へご連絡ください。

　・後日紛失した資格確認書が見つかった場合、すみやかに古い資格確認書をご返却ください。

　　なお、手数料の返却はいたしません。

　・資格確認書紛失時は必ず警察に連絡してください。

２．健康保険証の取り扱いについて

　2025年12月2日より、従来の健康保険証は廃止され、今後はマイナ保険証または資格確認書による受診となります。現在お持ちの黄色い健康保険証は返却の必要はありません。個人情報が表面に記載されていますので、各自で適切な方法により破棄していただきますようお願いいたします。

※なお、2025年11月30日までにご退職される方につきましては、健康保険証を事業主へご返却くださいますようお願いいたします。

※厚生労働省から、2026年3月末までの暫定措置として、12月2日以降医療機関等で保険証や「資格情報のお知らせ」を持参した場合でも、資格が確認できれば保険診療で受診できるという通知がありました。この措置は医療機関・薬局の特例的な対応であり、保険証が有効になるわけではありません。受診の際は、必ずマイナ保険証（または資格確認書）をご利用ください。

Introduction of a Fee for Reissuing Lost Eligibility Confirmation Document &

 

Handling of Old Health Insurance Cards

Thank you for your continued understanding and cooperation in the administration of the EY Health Insurance Association. We would like to inform you of two important changes we are introducing.

1. Fee for Reissuing Lost Eligibility Confirmation Document

At the 41st EY Japan Health Insurance Association meeting held on July 24, 2025, members approved the introduction of a fee for reissuing Eligibility Confirmation Document. The fee will apply from December 2, 2025. A reissuance fee will apply except when a certificate is lost due to a disaster.

(1) Start date

Tuesday, December 2, 2025 (applies to applications submitted on or after this date)

(2) Fee

1,000 yen per person (bank transfer fees to be borne by the applicant)

(3) Procedure for Reissuing a Eligibility Confirmation Document (Health Insurance Eligibility Certificate)

　1) Fill out an Application for Eligibility Confirmation Document Reissuance.

　2) Transfer the reissuance fee (1,000 yen) to the designated Health Insurance Association account

　using the insured person’s name as 　registered on the family register.

　*You do not need to attach a copy of the bank transfer receipt to your application.

　3) Please send the completed reissuance application form to the designated office.

　4) Once your application has been received and as long as there are no errors, your Eligibility

　Confirmation Document will be reissued.

　5) The reissued Eligibility Confirmation Document will be sent by registered mail to your address via

　the designated office. Please allow 　　

　two to three weeks for delivery.

(4) Important notes

　·The reissuance fee will be waived if the certificate was lost due to a disaster. A disaster certificate

　must be attached when applying in 　such cases.

　·Certificates will be reissued once your application has been received and payment confirmed.

　·Receipts will not be issued. Please contact the EY Japan Health Insurance Association if you need a

　receipt.

　·If you later find your lost certificate, please return the old one promptly. Please note that the

　reissuance fee is non-refundable.

　·If you lose your Eligibility Confirmation Document, please be sure to notify the police. 

2. Handling of Health Insurance Cards

From December 2, 2025, the current health insurance cards will be abolished and you will need to present either your My Number Card or Eligibility Confirmation Document when receiving medical care. You do not need to return your current yellow health insurance card. However ,since personal information is printed on the front of the card, please dispose of it properly.*If you are retiring on or before November 30, 2025, please return your health insurance card to your employer.*As a temporary measure announced by the Ministry of Health, Labour and Welfare, until the end of March 2026, if you bring your health insurance card or the “Eligibility Notification” to a medical institution or pharmacy, you may receive insured medical care as long as your eligibility can be confirmed. Please note that this is a special arrangement for medical institutions and pharmacies and does not make the health insurance card valid again. When receiving care, you must use your Individual Number Health Insurance Card (or Eligibility Confirmation Document).
