 資格確認書の一斉交付について

日頃より、健康保険組合業務運営にご理解、ご協力いただきありがとうございます。

現在お持ちの黄色い保険証は、2025年12月1日(令和7年12月1日)まで使用できますが、12月２日（令和7年12月2日）以降は使えなくなります。医療機関や薬局では、原則として健康保険証の利用登録をしたマイナンバーカード（以下「マイナ保険証」という。）で受診してください。

マイナ保険証を利用できない方には、健康保険証の代わりとして「資格確認書」をお送りいたします。申請のお手続きは不要です。健康保険組合にてマイナ保険証の利用登録状況を確認し、対象者へ送付いたします。

１．資格確認書一斉交付の対象者

2024年11月30日（令和6年12月1日）以前に資格取得された方（入社、被扶養者追加、任意継続加入等により黄色い保険証をお持ちの方）で、以下いずれかに該当する場合

①マイナンバーカードを作成していない

②マイナンバーカードはあるが、保険証利用登録をしていない

③マイナンバーカードを返納した

④電子証明書の有効期限が切れている

２．発送時期

10月中旬頃より順次ご自宅宛に簡易書留にて発送いたしますので、必ずお受け取りくださいますようお願いいたします。郵送時期に関するご要望はお受けいたしかねます。ご不在の場合には不在通知が入りますので、速やかに再配達の手続きを行ってください。

３．送付物

資格確認書（ハガキサイズ・紙）、有効期限2028年11月30日（令和10年11月30日）

【注意事項】

・医療機関等に資格確認書を持参しても、貴殿のマイナンバーと健康保険の資格が結びついていないと、医療費等の10割負担を求められる可能性もあります。資格の結びつけには、法人/会社へのマイナンバー提出が必要です。未登録の方はBulasマイナンバーWEB登録サービスからご家族分も含めたマイナンバー登録をお願いいたします。

・データ作成の都合により、今後9月～10月の間に退職される方にも資格確認書が届く場合があります。その際は、10月中旬～11月中旬頃健康保険組合から別送いたします返送用封筒にてご返却をお願いいたします。お手数をおかけしますが、ご対応のほどよろしくお願いいたします。

・資格確認書交付後マイナ保険証の利用を開始された方は、当該資格確認書は返却いただく必要ございません。個人情報を含みますので、適切な方法によりご自身で破棄してください。

・EY Japan健保に加入しているご家族に資格確認書の対象者がいる場合は、被保険者ご本人宛にまとめて送付します。なお、ご家族全員がマイナ保険証の利用登録をされている場合、送付物はありません。

・2024年12月1日（令和6年12月1日）以降に資格取得された方（入社、被扶養者追加、任意継続加入等により資格確認書をお持ちの方）の資格確認書再交付要領については、別途ご案内いたします。

 

Mass Issuance of Eligibility Confirmation Document

Thank you for your continued understanding and cooperation in the operation of the EY Health Insurance Society.

 

You may continue to use your current yellow health insurance card until December 1, 2025. From December 2, 2025, it will no longer be valid. In principle, when visiting a medical institution or pharmacy, please use your Individual Number Card that has been registered for use as a health insurance card (“Individual Number Health Insurance Card”).

 

If you are unable to use a Individual Number Health Insurance Card, we will send you a Certificate of Eligibility that can be used instead. No application is required to receive a certificate. We will confirm your Individual Number Health Insurance Card registration status and send certificates to those unable to use a Individual Number Health Insurance Card.

 

1. Persons who will receive a Certificate of Eligibility

Persons who acquired eligibility on or before November 30, 2024 (who have a yellow insurance card due to employment, addition of dependents, or voluntary continuation of membership) who meet any of the following conditions:

(1) Do not have a Individual Number Card

(2) Have a Individual Number Card but have not registered to use it as a health insurance card

(3) Have returned their Individual Number Card

(4) Whose digital certificate has expired

 

2. Timing

From mid-October, we will start sending out certificates by recorded delivery to your home address. Please be sure to receive it. We cannot accommodate requests regarding the mailing schedule. If you are not at home, a delivery notice will be left. Please promptly arrange for redelivery.

 

3. Contents of the mailing

Certificate of Eligibility (paper, postcard size), valid until November 30, 2028

 

Important Information

·      Even if you take a Certificate of Eligibility to a medical institution, if your My Number (personal identification number) is not linked to your health insurance eligibility, you may be required to pay for your medical expenses in full. To link your eligibility, you must submit your My Number to your employer. If you have not yet registered your My Number, please use the  Bulas My Number Web Registration Service to register your number and those of your family members.

·      Due to the timing of data preparation, some Eligibility Confirmation Document may be sent to those who retire in September or October. In such cases, please return the certificate using the return envelope that will be sent separately from us around mid-October to mid-November. We apologize for the inconvenience and thank you in advance for your cooperation.

·      If you start using a Individual Number Health Insurance Card after receiving a Certificate of Eligibility, you do not need to return the certificate. Since your certificate contains personal information, please be sure to dispose of it properly yourself.

·      If any of your family members enrolled in EY Japan Health Insurance qualify to receive a Certificate of Eligibility, it will be sent together with yours to you as the insured person. Please note that if all family members have registered to use their Individual Number Card as a Individual Number Health Insurance Card, nothing will be sent.

·      The procedure for reissuing Eligibility Confirmation Document for those who acquired eligibility on or after December 1, 2024 (such as through employment, addition of dependents, or voluntary continuation of membership) will be announced separately.
