Lost Health Insurance Card
Insured persons and dependents need to handle their health insurance card with care because the EY Japan Health Insurance Society cannot freeze the health insurance card even if lost or stolen. Please immediately notify the police in the event of a lost or stolen health insurance card.
Please also make sure to complete the procedures to reissue a health insurance card. Insured persons with optional continued insurance should contact the EY Japan Health Insurance Society.
Procedures
If you lose or stain (damage) your health insurance card, please immediately submit the “Application for Reissuance of Insured Individual Card (Elderly Beneficiary Health Insurance Card) Due to Loss (Damage).”
