Death

The EY Japan Health Insurance Society pays funeral expenses (family member burial expenses) if an insured person or dependent dies. Please make sure to complete the procedures for the loss of health insurance eligibility.

Additional benefits from the EY Japan Health Insurance Society ・Additional burial fee: 50,000 yen provided
・Additional charge for family burial expenses: 40,000 yen paid

Death of an Insured Person

The EY Japan Health Insurance Society pays a uniform 50,000 yen for the funeral expenses in the event an insured person dies and is buried by family.
If the insured person does not have any family, the person who handled the actual burial receives burial costs up to 50,000 yen as funeral expenses.

Death of a Dependent

The EY Japan Health Insurance Society pays a uniform 50,000 yen for family member burial expenses in the event a dependent dies.

Procedures

Please enter the required items on the Application Form for Payment of Burial Fees (Expenses)/Surcharge and submit it together with the required attached documents. (Refer to the Miscellaneous Application Forms page for submission destinations.)
Please make sure to complete the procedures below.

Required Documents
Death of an Insured Person
  • Document certifying death (either employer certification, copy of death certificate, copy of burial permit, copy of cremation permit, etc.)
  • Return the health insurance card (notification by employer)

*Please attach the following if applicable

[Funeral expenses: if the claimant is not a dependent of the EY Japan Health Insurance Society]
  • Documents to verify financial support (documents that include the deceased insured person and the claimant, such as a certificate of residence)
[Funeral fees: if claimed by the person who actually carried out the burial]
  • Receipt for expenses incurred for burial (original copy addressed to the name of the claimant)
Death of a Dependent
  • Document certifying death (either employer certification, copy of death certificate, copy of burial permit, copy of cremation permit, etc.)
  • Attach the health insurance card to the Notification of Change of Dependents
