Marriage
If the insured person gets married and changes his/her name or address, procedures are required. Please inform the business operator.
In addition, procedures are required to add the new spouse as a dependent or even to remove a dependent who has married.
Procedures Related to Marriage
| Name change |
We will issue you an Eligibility Notification, an Eligibility Confirmation Document, etc., after your name change, based on the information provided by company. Please check the company intranet for information on how to notify your employer.Please return the old insurance card, Eligibility Confirmation Document, etc., to company.
* Even if you plan to continue using your maiden name at work, you will need to go through the process of changing your name.
* After December 2, 2024, new health insurance cards will no longer be issued.

Submit a Notification of Change of Dependents within five days.

| Removing a dependent (family member) as a dependent after marriage |
Submit a Notification of Change of Dependents together with the health insurance card, the Eligibility Confirmation Document, etc., if you have it within five days.

* There is no need to return your Individual Number Insurance Card.
