Marriage
If the insured person gets married and changes his/her name or address, procedures are required. Please inform the business operator.
In addition, procedures are required to add the new spouse as a dependent or even to remove a dependent who has married.
Procedures Related to Marriage
Name change |
Please notify the business operator immediately and submit your health insurance card together with the notice. The health insurance society will issue a health insurance card after the name change based on information from the business operator. *Even if you continue to use your maiden name in your company, you must change your name on your health insurance card.

Adding a spouse as a dependent |
Submit a Notification of Change of Dependents within five days.
