2025/12/04 資格確認書の継続交付について
日頃より、健康保険組合業務運営にご理解、ご協力いただきありがとうございます。
健康保険証は2025年12月2日(令和7年12月2日)以降無効となり、原則マイナ保険証(健康保険証として利用登録されたマイナンバーカード)へと移行されますが、今後、マイナ保険証の利用登録が確認できない被保険者および被扶養者には随時、保険証の代わりとなる『資格確認書』を交付します。
1.交付対象者
(1)お手元の資格確認書の有効期限が切れており、利用登録状況が以下いずれかに該当する方
①マイナンバーカードを作成していない
②マイナンバーカードはあるが、保険証利用登録をしていない
③マイナンバーカードを返納した
④マイナンバーカードの電子証明書の有効期限が切れている
(2)マイナ保険証の利用登録をしていたが、下記に該当する方
①マイナンバーカードを返納した方
②保険証利用登録を解除した方
③マイナンバーカード/電子証明書の有効期限が切れた方
2.発送時期
上記1.交付対象者別の発送時期は、下表をご確認ください。

※ご自宅宛に特定記録にて発送いたします。郵送時期に関するご要望はお受けいたしかねます。
3.送付物
資格確認書(ハガキサイズ・紙)、有効期限2028年11月30日(令和10年11月30日)

【注意事項】
・医療機関等に資格確認書を持参しても、貴殿のマイナンバーと健康保険の資格が結びついていない
・マイナ保険証を利用予定の方でも利用登録状況の確認タイミングの都合で更新用の資格確認書が届く場合がございます。
・マイナ保険証の利用を開始された方は、当該資格確認書は返却いただく必要はございません。個人情報を含みますので、適切な方法によりご自身で破棄してください。
・EY Japan健保に加入しているご家族に資格確認書の対象者がいる場合は、被保険者ご本人宛にまとめて送付します。
Continued Issuance of Eligibility Confirmation Document
Thank you for your continued understanding and cooperation in the operation of the EY Health Insurance Society.
Your health insurance card will become invalid after December 2, 2025, and, in principle, will be replaced by your Individual Number Health Insurance Card (a My Number Card registered for use as a health insurance card). Insured persons and dependents whose registration for use of the Individual Number Health Insurance Card cannot be confirmed will be issued with an Eligibility Confirmation Document for use in place of a health insurance card.
1. Persons who will Receive An Eligibility Confirmation Document
(1) Persons whose current Eligibility Confirmation Document has expired and whose registration status falls under one of the following categories:
1) Do not have a My Number Card
2) Have a My Number Card but have not registered to use it as a
health insurance card
3) Have returned their My Number Card
4) Have a My Number Card whose digital certificate has expired
(2) Persons who had registered to use the Individual Number Health Insurance Card but fall under one of the following categories:
1) Have returned their My Number Card
2) Have cancelled their registration for use of the
Individual Number Health Insurance Card
3) Have a My Number Card or digital certificate that has expired
2. Timing
Please refer to the table below for the timing of certificate mailings for each eligible category.
* Certificates will be sent to your home address by Specified Recorded Mail. Requests regarding the mailing schedule cannot be accommodated.
3. Items to be sent
Eligibility Confirmation Document (paper, postcard size), valid until November 30, 2028

[Important Information]
- Even if you present an Eligibility Confirmation Document at a medical institution, you may be temporarily required to pay your medical expenses in full if your My Number has not been linked to your health insurance eligibility. To complete this linkage, you must submit your My Number (personal identification number) to your employer. If you have not yet registered your My Number, please search “My Number Registration” on SharePoint for instructions on registering your number and those of your family members.
- Persons who plan to use the Individual Number Health Insurance Card may receive a renewal Eligibility Confirmation Document depending on the timing of registration status verification.
- Persons who have started using their Individual Number Health Insurance Card do not need to return the issued Eligibility Confirmation Document. As the certificate contains personal information, please dispose of it properly.
- If any of your family members enrolled in EY Japan Health Insurance are to receive an Eligibility Confirmation Document, their certificate will be sent together with yours to you as the insured person.
